Spring Registration!

Hi Tidal Wave Families-
Attached you will find the appropriate registration form for Spring Tidal Wave Soccer Registration. Once I register the entire team I will have a better idea on costs and will let you know asap.

Please fill out the form in BLUE ink and bring a copy to the game on Saturday. (I will also have copies available on Saturday if you are not able to fill it out prior to then.) Below are instructions for how to fill out the top portion of the form, you will need to add in the information below in BLUE. Thank you to those of you who have given me copies of your daughters birth certificate, all others, please bring this weekend so I can turn everything in together.
